INFORMED CONSENT FOR NUTRITION SERVICES
You understand that you are joining Empowered Eating Academy so that you can obtain information and guidance about health factors within your own control (diet, nutrition, and related behaviours) in order to support your general health and wellness. You understand that M. Oettle is providing support as a dietitian and coach - not a physician or psychologist, and that she does not dispense medical advice nor prescribe treatment. You also understand that through this program, M. Oettle does not provide Medical Nutrition Therapy (nutritional therapy aimed at treating diagnosed medical conditions). Rather, M. Oettle provides education to enhance your knowledge of health as it relates to food and behaviours associated with eating. You understand that Empowered Eating Academy is not a substitute for the diagnosis, treatment, or care of disease by a medical provider.
You have the sole responsibility to contact your physician for approval of participation in nutrition services / coaching. If you choose to use this information without such approval, you agree to accept full responsibility for this decision.
You understand that you are not to follow any recommendations provided through the Empowered Eating Academy program if these recommendations contradict recommendations given to you by your physician or medical team, in relation to a specific medical condition / allergy.
You recognize that any activity in which problems / life situations are discussed bears some risk, which you the client agree to accept in its entirety. In consideration of your participation in Empowered Eating Academy, you hereby accept all risk to your health, including injury or death that may result from such participation and you hereby release M. Oettle on your behalf and on behalf of your personal representatives, estate, heirs, next of kin, peers and assigns from any and all costs, claims, causes of action and damages arising from any and all illness or injury to your person, including your death, that may result from or occur as a result of your participation in the Empowered Eating Academy and coaching services by M. Oettle whether caused by negligence or otherwise.
You accept the risk of any decision, action or outcome based on the coaching relationship. You acknowledge that expectations and results or participation in coaching activities vary among individuals and that each individual may not receive the same benefit. Any recommendations you follow for changes in diet are entirely your responsibility. It is your responsibility as the client to disclose the nature of any disease, disorder or condition and all other relevant details to M. Oettle. If you have not previously consulted a licensed physician about this disease, disorder or condition, you acknowledge that it’s your responsibility to have this addressed, prior to joining Empowered Eating Academy. You are not to alter or discontinue treatments prescribed by a licensed physician or other licensed health professional without consulting the individual who prescribed the treatment.
The parties hereby acknowledge and agree that you, the client has not relied upon any warranties or representations not contained in this agreement. This agreement is the entire bargain between the parties. If any part of this agreement is held to be void or unenforceable then the parties are that the unenforceable or void clause will be severed from the agreement and the agreement will be held to be effective despite such severance, provided always that the agreement can still be effective notwithstanding that clause (as the case may be) being severed from this agreement.
PARTICIPATION IN EMPOWERED EATING ACADEMY
You have lifetime access to the course material and understand that this access is for you, the client, only and should not be shared with anyone. The information in Empowered Eating Academy may or may not be relevant or specific to you (or anyone else’s) individual needs and should only be used as a guide to assist your health and well-being. You know as a client you are responsible for the actions you take from the information, worksheets and extra resources within Empowered Eating Academy as well as anyone you share this information with.
You have access to Marna via & email if you need to be in communication with her. Marna will respond to all emails within 72 hours of their delivery.
PRIVACY
Client information and records are confidential, in line with the POPI act, unless I receive your advance permission to disclose or except as required by law. All your personal information recorded is confidential. See the privacy policy for more details.
POPI PERMISSIONS
The information you provide through signing up to this program will only be used in accordance with the requirements of the Protection of Personal Information (POPI) Act of South Africa and as detailed in the Privacy Policy (https://www.nutritionsmart.co.za/privacy-policy-2/).
AGREEMENT
I have carefully read this agreement and understood it to be a full and final release of all costs, claims, causes of action and damages of any kind arising from or in connection with Empowered Eating Academy and related coaching services from M. Oettle. I also understand that by clicking below, I am giving consent to collect and process my personal information in accordance with the POPI Act.
